Between 1999 and 2024, the league has achieved significant milestones in both local competition and the development of Major League stars.

The Tigres del Licey and the Águilas Cibaeñas have maintained their rivalry as the most successful teams in LIDOM history. Licey captured championships in 2002, 2004, 2009, 2014 (2013-14 season), 2021 (2020-21), and most recently in 2023 (2022-23) and 2024 (2023-24), reaching a team-leading total of 24 titles. The Águilas have kept pace with championships in 2001, 2003, 2007, 2008, 2017 (2016-17), 2018 (2017-18), and 2021 (2020-21), reaffirming their status as constant contenders.

In 2021, the Águilas Cibaeñas won the Caribbean Series , becoming the first team in history to win the regional title with an undefeated record (7–0). That same year, Robinson Canó, former Yankees and Mariners player, returned to LIDOM.

LIDOM has been instrumental in developing players who later excelled in the MLB. Puerto Rico's Roberto Clemente Professional Baseball League has resurfaced since 2013, thanks in part to the World Baseball Classic and the support of MLB. The Criollos de Caguas have been dominant in recent years, with championships in 2011, 2013, 2017, 2018, 2020, 2021, and 2024. In the Caribbean Series, Caguas won back-to-back titles in 2017 and 2018—the first for a Puerto Rican team in more than 20 years.

In Cuba , the National Series has maintained its prestige despite defections and internal reforms. Between 1999 and 2015, teams such as Industriales, Santiago de Cuba, and Pinar del Río won titles. Pinar del Río's 2014 championship resulted in the 2015 Caribbean Series title, marking Cuba's return to international competition. During that period, figures such as José Abreu, Yulieski Gurriel, and Yoenis Céspedes left the island and found success in the MLB.

The Colombian Professional Baseball League gained momentum after the success of Colombian players in the MLB. The Caimanes de Barranquilla have become the most successful team in recent years, with multiple national titles and the historic conquest of the Caribbean Series in 2022, the first for Colombia.

The Caribbean Series has expanded and now regularly includes between six and eight teams from the Dominican Republic, Mexico, Puerto Rico, Venezuela, Panama, and Colombia. The tournament has been key to the development of regional baseball and is a scouting showcase for MLB organizations. Since 2000, Dominican teams have won the Caribbean Series 12 times, while Mexico and Puerto Rico have also won multiple titles, reinforcing a closely matched regional rivalry.
